Best NEMT Software in Virginia for Transportation Providers

Virginia's Medicaid transportation system changed significantly on July 1, 2025. DMAS launched the Cardinal Care Managed Care contracts, consolidating the old Medallion 4.0 and CCC Plus programs under one structure. Five MCOs now manage most Medicaid members: Aetna Better Health of Virginia, Anthem HealthKeepers Plus, Humana Healthy Horizons in Virginia, Sentara Community Plan, and UnitedHealthcare Community Plan.

For NEMT providers, this transition shifted broker relationships. At the Fee-for-Service level, Modivcare remains DMAS's contracted statewide broker. At the MCO level, Anthem uses Access2Care. Humana members are directed to Modivcare for FFS-administered trips. UnitedHealthcare Virginia moved its NEMT to SafeRide Health on March 1, 2026. Providers who were credentialed with Access2Care for UHC trips had to re-enroll with SafeRide, learn a new portal, and meet new KPI thresholds.

On top of that, every NEMT carrier in Virginia must hold a Non-Emergency Medical Transportation Certificate from the Virginia DMV, maintain a $25,000 surety bond, and ensure every driver completes PASS certification before transporting any Medicaid member. Drivers with a barrier crime conviction under Virginia Code Section 37.2-314(B) cannot operate.

How NEMT Software Works in Virginia

Virginia's dual-track system sends trips through two separate channels. FFS trips come through Modivcare with a standard five-business-day advance reservation window. MCO trips come through broker-specific portals depending on which MCO the member is enrolled with. A provider contracted with multiple brokers checks multiple portals every morning unless software consolidates them.

  • Trip Assignment Across FFS and MCO Brokers. FFS trips come through Modivcare. Anthem MCO trips come through Access2Care. UHC trips now come through SafeRide Health. Sentara runs its own NEMT coordination line. Software pulls all of these into one dispatch queue. Your dispatcher sees every assignment in one place from the start of each shift.
  • Dispatch Across Urban and Rural Virginia. Northern Virginia runs dense urban schedules with tight appointment windows and high vehicle utilization. Southwest Virginia counties like Wise, Lee, and Tazewell involve long rural routes where one driver might cover 80 or more miles for two or three trips. Route optimization handles both scenarios, but the logic is different. Software manages that distinction automatically.
  • Trip Verification. DMAS's April 2025 driver and vehicle requirements specify that every vehicle must have functioning GPS. Broker KPIs require real-time location verification, geofenced pickup and drop-off events, and electronic signatures. The software captures all of this during the trip. Your drivers do not fill out paper logs.
  • Billing. Virginia's timely filing limit with DMAS is 12 months. Broker payment cycles run 14 to 30 days from a clean submission. The platform generates billing records directly from trip logs, runs pre-submission error checks, and sends clean claims the first time.

The Cardinal Care Transition and Broker Changes

The July 2025 Cardinal Care rollout brought new MCO contracts and, in UHC's case, an entirely new broker. SafeRide Health replaced Access2Care for all UHC Virginia members effective March 1, 2026. Providers had to re-enroll, configure new portal access, and meet SafeRide's published KPI thresholds: 100% ride response rate, 95% ride acceptance rate, 95% on-time performance, fewer than 1% complaints, and fewer than 5% kickbacks. Without organized records and systems, that kind of transition is difficult to manage.

Strict Driver and Vehicle Standards

DMAS imposes specific requirements for every driver transporting Medicaid members. PASS certification is mandatory before a driver's first trip. Barrier-crime background checks must be current. Vehicles must maintain 68 degrees Fahrenheit cooling and 74 degrees Fahrenheit heating in the passenger compartment, carry a seat-belt cutter within reach of the driver, have functioning GPS, and display required safety signage. Tracking these compliance dates across a fleet without a system produces gaps.

Insurance Costs in Northern Virginia

Virginia DMV requires $350,000 minimum liability for vehicles carrying 1 to 6 passengers, $1,500,000 for 7 to 15 passengers, and $5,000,000 for 16 or more passengers. In Northern Virginia, where commercial insurance premiums are among the highest in the state, those requirements translate into $7,500 to $11,000 per vehicle annually for a new operator. Your billing volume needs to justify that cost, which requires accurate trip data.
