How to Start an NEMT Business in New Jersey

New Jersey's Medicaid program, NJ FamilyCare, covers NEMT statewide through Modivcare. This means one broker, consistent reimbursement, and predictable authorization. But it also means competition: hundreds of operators bid for the same trips. Success requires efficiency, reliability, and speed.

This guide covers New Jersey-specific licensing, costs, and scaling strategies.

What Is NEMT and How It Works in New Jersey

Modivcare brokers all NJ FamilyCare (Medicaid) NEMT. Members request trips, Modivcare approves or denies them by applying a 20-mile distance limit, and assigns approved trips to available providers. You get paid $3.50–$8 per mile depending on vehicle type and trip complexity. Volume and efficiency drive profit.

Unlike fragmented states, New Jersey is simple: one broker, one authorization process. This favors quick, efficient operators.

Requirements in New Jersey

Business Registration and Licensing

New Jersey doesn’t require a separate NEMT license, but you must be registered as a business and approved by Modivcare.

  • Register with the NJ Department of State as an LLC or corporation.
  • Obtain an EIN from the IRS.
  • Register for NJ sales tax (if required).
  • Apply to Modivcare as an approved provider (they’ll assign you a provider ID).

Timeline: 3–5 weeks. Cost: $100–$150 for state registration.

Insurance

Modivcare requires comprehensive insurance before credentialing.

  • Commercial auto liability: $1 million per accident. Cost: $4,500–$9,000/year for 3–5 vehicles.
  • General liability: $1 million per occurrence. Cost: $1,000–$1,500/year.
  • Workers’ compensation: Mandatory for employees. Cost: ~14% of payroll.

Total first-year insurance: $5,500–$10,500.

Vehicles

Start with ambulatory vehicles. NJ has no wheelchair/stretcher requirement like some states, but offering wheelchair service expands your market.

  • Ambulatory sedan: Lease $350–$450/month or buy for $10,000–$18,000.
  • Wheelchair van: Lease $800–$1,000/month or buy for $35,000–$50,000.

All vehicles must have GPS for billing verification and pass NJ emissions standards.

Drivers

Modivcare requires:

  • Valid New Jersey driver’s license (CDL for specialty vehicles).
  • Clean background check (3+ years driving record, no moving violations).
  • Proof of medical clearance (physical fitness to drive).

Typical wage: $19–$25/hour, including benefits (NJ minimum wage supports a higher baseline).

Setup Timeline

-

Step-by-Step Setup

From registering your business to your first live trip typically takes about ten weeks. Here is how the process breaks down.

  • Step 1: Register Your Business (Weeks 1–3). Create an LLC or corporation with the NJ Department of State. Obtain an EIN from the IRS. This is your legal foundation.
  • Step 2: Secure Insurance (Weeks 2–4). Apply through an insurance broker. You'll need certificates of insurance for your Modivcare application.
  • Step 3: Apply to Modivcare (Weeks 3–5). Contact Modivcare's provider relations line. They'll send a credentialing application. You'll submit your business registration certificate, insurance certificates, owner/operator background information, driver background clearances, and banking and tax information. Timeline: 4–6 weeks to approval. Modivcare credentialing is your gate to revenue.
  • Step 4: Lease Vehicles and Install GPS (Weeks 4–6). Lease 2–3 ambulatory vehicles. Install GPS trackers ($500–$800 per vehicle; leasing companies often bundle this). GPS is required for billing verification.
  • Step 5: Hire and Vet Drivers (Weeks 5–8). Recruit drivers with clean records. Run background checks immediately since they take 4–6 weeks. While waiting, train on customer service, GPS usage, and member privacy.
  • Step 6: Go Live (Week 10+). Once Modivcare approves and drivers are cleared, you receive trip assignments. Ramp-up: 10–30 trips in week one, accelerating to 150+ by month two.

Common Challenges

Urban Competition

Newark, Jersey City, and other dense areas have dozens of providers. Speed and reliability differentiate you—respond to Modivcare assignments within 15 minutes.

The 20-Mile Rule

Understand it thoroughly. Trips beyond 20 miles are denied unless exceptions apply. Misunderstanding the rule means wasted time and no revenue.

No-Show Liability

Each no-show costs roughly $40–$80 in Medicaid clawback. Tight confirmation systems and reliable drivers are mandatory.

Fuel Costs

New Jersey's gas prices are high. Tight routing reduces fuel waste.

Frequently Asked Questions

How long until I’m profitable?

Typically 3 to 5 months. Month one runs 20–40 trips. Month two reaches 80–100 trips. Month three reaches 150+ trips. By month four, margins improve as overhead spreads across more trips.

Is the Newark area a good market?

Yes. It has the highest density, highest Medicaid population, and consistent trip volume. Urban providers see faster ramp-up than suburban ones.

What if Modivcare rejects my application?

Rare if you have clean backgrounds and valid insurance. If denied, appeal or reapply after 6 months. You can start private pay while waiting.

Can I work with both Modivcare and MCOs?

Modivcare is the primary Medicaid broker in NJ. Some MCOs, such as Horizon and UnitedHealthcare, manage their own NEMT, but Modivcare is your main revenue source.

How much can I make?

With 3 vehicles and 250+ trips/month, gross revenue runs $8,750–$16,250/month. After costs of roughly $8,000–$10,000, net profit is $750–$6,250. Margins improve with scale.

Do I need a dispatch office?

No. Start from home. As you scale, a small office helps driver coordination, but it’s not required initially.
