Running NEMT in Florida means working inside one of the most broker-heavy Medicaid systems in the country.

Florida's Statewide Medicaid Managed Care (SMMC) 3.0 program launched February 1, 2025 under AHCA. It routes Medicaid members through eight managed care organizations across nine geographic regions. Each MCO picks its own transportation broker. In 2026, Florida providers may receive trips from Modivcare, MTM Health, Alivi, or Ride2MD, depending on which MCO the member belongs to and which region you operate in.

MTM Health took over from Access2Care for UnitedHealthcare Florida members on March 1, 2026. Molina Healthcare Florida followed on March 12, 2026. Alivi has handled Sunshine Health trips since January 2025. Florida Community Care uses Ride2MD statewide.

A provider covering multiple regions can be active with three or four brokers at once. Each broker has its own portal, its own trip-assignment system, and its own billing process. Modivcare's Financial Restructuring

Modivcare, which handles trips for Humana, Simply Healthcare, Aetna, Community Care Plan, and AHCA fee-for-service members in Florida, filed Chapter 11 in August 2025 and emerged on December 29, 2025 as a privately held company with approximately $300 million in funded debt. Services continued without interruption. Expect tighter performance enforcement and faster technology-driven accountability from Modivcare going forward. Accurate trip documentation and on-time compliance matter more now, not less.

Dispatch Without Automation

A dispatcher managing 40 or more daily trips manually is near capacity. Incoming broker assignments, vehicle availability, driver schedules, and trip confirmations all require attention at the same time. One missed portal update or unanswered assignment can trigger a performance flag.

Billing Rejections

Florida Medicaid's timely filing limit is 12 months, but broker payment cycles run 14 to 30 days from a clean submission.
